Prefab damper repair
In the IHP guide the damper is a metal plate at the base of the chimney with two positions, open and closed. On one Superior line the handle sits in the upper front face of the unit and moves through a keyway slot, and a weight helps the blade swing open when the handle releases it.
Rust, bent linkages and worn handles can leave a damper stuck halfway. Attic insulation shields around the chimney
Section R1005.8 of the Florida Building Code deals with the spot where a factory-built chimney meets insulation. It requires a shield of steel no thinner than 26 gauge that holds the insulation away from the pipe by the chimney maker's clearance, and in an attic the shield has to rise 2 inches or more above the insulation and be fastened so it stays put. One Superior manual also warns never to fill the chimney enclosure with blown insulation.

The outside air kit on a prefab fireplace
On a factory-built fireplace the outside air kit is part of the listing. Section R1006.1.1 of the Florida Building Code requires the combustion air duct for these units to be a listed component installed to the fireplace maker's instructions. One Superior manual adds that the inlet duct must not end in an attic and that the air inlet must sit at least 3 feet below the flue gas outlet at the top of the chimney.
