Finding the fireplace fault before ordering parts
A symptom shows where to start looking. It does not tell you what to replace. Smoke at the opening can come from a closed or broken damper, a narrowed passage or a house pulling air the wrong way. Crumbling material on the hearth can fall from the firebox, the smoke chamber or a factory-built panel. Each of those has a different repair and a different price.

Outside air supply repair for a fireplace
A fire needs air, and a tightly sealed house can starve it. The Florida Building Code asks for an exterior air supply for fireplaces, except in rooms where mechanical ventilation keeps indoor pressure neutral or positive (R1006.1). The intake is covered with 1/4 inch corrosion-resistant mesh, and the outlet at the firebox has to close (R1006.2, R1006.5).
Outside air kits rust shut, lose their screens or get blocked by insects and yard debris. On a factory-built unit, the duct is a listed part of the fireplace, so the replacement comes from the maker.
Listed parts for factory-built fireplace repair
A factory-built fireplace and its chimney were tested together as a system. Swapping in a generic grate, a different maker's chimney section or a home-made heat shield changes the assembly that was tested. The Florida Building Code holds factory-built chimneys to their listing and the maker's installation instructions as well (R1005.1).
